STEP 1: Read over all of the info regarding training:

It is important to be informed about the process from start to finish! Here is some light reading material to peruse before moving on to step #2:

STEP 2: Request for Training:

Ready to have your pup become the best it can be? Please fill out the following form after deciding that you would like to proceed with scheduling a training session with Katie Fern.
(If you have more than one dog, please fill out one form per dog).

STEP 3: Fill out and turn in forms:

Already secured a spot for training? Here are the documents you'll need at drop off! Please Print, Sign, and bring these forms with you when dropping off a dog for training! 
  • Training Request Form 
  • Liability Waiver (printed and signed)
  • Photo Release Form (printed and signed) Feel free to print, scan, and email these forms ahead of time as well!
  • Proof of recent negative Fecal test (within the past month for puppies, within 6 months for dogs older than 9 months)

STEP 4: Get your pup ready for his/her "vacation"!

What do I need to bring to training?
  • Food for entire stay (labeled with your dog's name)
  • A placemat to use for training (i.e. beach towel, doormat, or rug, please NO dog beds. Size: just enough room for them to turn around and lay down) with your dog's name labeled on it
  • Copies of all vaccination records (Rabies, Distemper, and puppy vaccinations)
  • Liability and Photo Release forms printed and signed

*We have many toys, bedding, bowls, and crates, so please don't worry about bringing these. Feel free to include any treats and bones if you'd like!
